Whooping cough cases increase

Cases of whooping cough (pertussis) reported to the Health Protection Agency (HPA) in England and Wales have more than doubled from 421 cases in 2010 to 1040 in 2011. Increases in levels of whooping cough are seen every three to four years and figures in 2011 are in line with cases reported in the last peak year of 2008.

You are able to request repeat prescriptions by:-

using the right hand tear off repeat medication sheet received each time you order repeat prescriptions - this can be handed in or posted to the practice at any time. (Please note: any medications you use that DO NOT appear on this printed list must be ordered using the Dunblane Medical Practice Special Request Form).

completing the online Repeat Prescription Form which can be accessed via the order link above.

You are able to request any prescriptions which you use that DO NOT appear on your printed medication list by:

completing the Prescription Special Request Form available from reception or which can be printed HERE and handed in or posted to the surgery.
completing the online prescription form using the Comments section to enter ALL the same details requested by the GP on the Prescription Special Request Form.

ANY ITEMS OF MEDICATION WHICH YOU USE THAT DO NOT APPEAR ON YOUR REPEAT PRESCRIPTION LIST MUST BE ORDERED USING THE INFORMATION REQUIRED ON THE SPECIAL REQUEST FORM.

Please note we do not accept requests for repeat prescriptions by telephone. Please allow 48 hours before collection. Bannerman's pharmacy also provides a delivery service for those patients unable to collect their prescriptions
